CGBC Event Release/Waiver Form:  This agreement releases Cedar Grove Baptist Church from all liability relating to injuries that may occur during any and all activities at Cedar Grove Baptist Church, located at 2485 Cedar Grove Road Lebanon, TN 37087 or off campus during organized church functions or events. By signing this agreement, I agree to hold Cedar Grove Baptist Church entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence or any other means.   I also acknowledge the risks involved in activities of Slip-n-Slide, water related activities, playgrounds, etc, or any dangerous activities off campus. These include but are not limited to falls, scrapes, sunburn, broken bones and any major trauma or medical issues.  I affirm that my child is participating voluntarily, and that all risks have been made clear to me.  By signing below, I forfeit all right to bring a suit against Cedar Grove Baptist Church for any reason. I will also make every effort to obey safety precautions as listed in writing and/or explained to me verbally during any event. I will ask for clarification when needed.
